Case Management Nurse – Bureau of Health Services

About the role

The Fire Department of New York (FDNY) Bureau of Health Services is looking for a full‑time Case Management Nurse to manage medical qualification, annual medical exams, and clinic services for firefighters, EMS candidates, and department incumbents.

Key responsibilities

• Maintain a comprehensive case‑management system for medical qualification and annual exams.
• Assess patient needs using clinical tools and data, and create individualized treatment plans.
• Determine eligibility for service‑connected coverage and clearly explain prescribed treatments.
• Perform nursing assessments, educate patients on benefits and resources, and communicate with physicians and insurers.
• Document complex medical information in the electronic medical record (EMR) and follow regulatory and quality‑management processes.
• Review medical test results, select appropriate external providers, and schedule appointments.
• Conduct telehealth visits for screening, including COVID‑19 protocols.
• Perform intramuscular, intradermal, subcutaneous injections and venipuncture as required.

Required profile

• Valid New York State Registered Nurse license.
• Bachelor’s degree in nursing from an accredited college.
• Minimum three years of nursing experience, preferably in case management or occupational health.
• Ability to work independently and follow departmental policies.
• Strong communication skills for interaction with patients, physicians, and insurance companies.
