Outpatient Registered Nurse (RN)

About the role

The Outpatient Registered Nurse (RN) provides entry‑level, clinically competent care to patients receiving hemodialysis. Working within the End Stage Kidney Disease (ESKD) team, the RN assesses, plans, intervenes, and evaluates patient needs while supporting quality improvement initiatives.

Key responsibilities

• Collect, analyze, and document patient data before, during, and after hemodialysis treatments.
• Adjust treatment plans as needed and notify the Team Leader, Charge Nurse, Supervisor, or Physician.
• Assess and educate patients and families on dialysis, transplant options, and care plans.
• Coordinate daily safe delivery of hemodialysis therapy with direct and ancillary staff.
• Administer medications per prescription or approved algorithms and document outcomes.
• Initiate emergency response measures when required.
• Serve as a resource for the care team and assist with staff training and orientation.
• Ensure proper laboratory collection, processing, and shipping.
• Maintain up‑to‑date nursing assessments and care plans in collaboration with the interdisciplinary team.
• Complete Clinical Advancement Program (CAP) requirements for professional growth.

Required profile

• Current Registered Nurse (RN) license.
• Entry‑level designation (CAP 1) within the Clinical Advancement Program.
• Ability to assess, plan, intervene, and evaluate patient care.
• Strong documentation skills and attention to detail.
• Capability to delegate tasks to LVN/LPNs and Patient Care Technicians.
