Primary Care Physician (PCP) – Full‑Risk Medicare Advantage

About the role

ChenMed is seeking dedicated Primary Care Physicians to join a rapidly expanding, physician‑led organization that delivers value‑based Medicare Advantage care to seniors. As a PCP you will manage a panel of up to 450 patients, act as a health coach, and drive outcomes that improve both health and cost.

Key responsibilities

• Provide high‑quality, cost‑effective clinical care for a panel of up to 450 patients.
• Coach patients to set and achieve short‑ and long‑term health goals, following up regularly.
• Simplify care plans to make behavior change actionable for patients.
• Meet and exceed performance metrics such as patient admissions per thousand, appointment slot utilization, CGCAHPS scores, clinical gap closures, and medical cost targets.
• Collaborate with care teams, managers, and medical directors to ensure compliance with guidelines and participate in risk‑ and quality‑management programs.
• Champion a positive culture of love, accountability, and passion within the medical center.
