Pharmacist Consultant – Disease Management

About the role

Elevance Health is seeking a Pharmacist Consultant to support disease‑management programs and physician care plans. The role focuses on drug‑therapy evaluation, member education, and collaboration with clinical teams to improve health outcomes.

Key responsibilities

• Review medication histories to assess appropriateness of prescriptions.
• Contact members and physicians to evaluate compliance and identify barriers.
• Provide education on proper medication use and suggest alternatives when therapies do not meet national standards.
• Collaborate with physicians to discuss medication adjustments and counsel on alternative options.
• Serve as a clinical resource for disease‑management initiatives and other therapeutic areas.

Required profile

• BA/BS in Pharmacy (PharmD preferred) with a minimum of 2 years of clinical experience.
• Current, active, unrestricted RPh license.
• Residency in Louisiana and ability to commute to the Metairie office.
• Experience with direct patient counseling, education, and managed‑care settings is highly preferred.
