Ambulatory Pharmacist – Primary Care

About the role
The Ambulatory Pharmacist is a patient‑centered clinical professional who works within primary‑care clinics to optimize medication therapy, reduce polypharmacy, and improve chronic‑disease outcomes. Operating under a collaborative practice agreement, the pharmacist partners with physicians, advanced practice providers, nurses, and care teams to enhance medication safety, efficacy, and cost‑effectiveness.
Key responsibilities
- Collaborate with providers to adjust medication regimens, interpret lab results, and support clinical decisions.
- Conduct comprehensive medication reviews addressing safety, efficacy, cost, side effects, and adherence barriers.
- Manage chronic conditions such as diabetes, hypertension, anticoagulation, and asthma.
- Facilitate care transitions, resolve medication discrepancies, and help prevent readmissions.
- Provide personalized patient education on medication use, side effects, and adherence strategies.
- Order, interpret, and act on laboratory tests to monitor medication safety and effectiveness.
- Implement and follow care protocols, including independent medication adjustments when authorized.
- Support physician consultations, supply accurate drug information, and contribute to quality‑improvement initiatives.
Required profile
- Knowledge of state and federal insurance programs and pharmacy‑related regulations.
- Ability to work independently and as part of a multidisciplinary team.
- Excellent organizational, communication, and clinical decision‑making skills.
- Strong attention to detail and capacity to manage multiple priorities.
- Willingness to travel locally up to 50% and flexibility to work evenings, weekends, or holidays as needed.
