Virtual Patient Care Coordinator

About the role

The Virtual Patient Care Coordinator joins Health & Virtuals to ensure smooth, predictable workflows for healthcare practices across the United States. Working remotely, you will act as the central hub for patients, providers, and external partners, handling administrative tasks that keep care moving without delays.

Key responsibilities

• Verify patient insurance coverage, including copays, deductibles, and coinsurance.
• Initiate, track, and follow up on prior authorizations for services, labs, imaging, procedures, and prescriptions.
• Submit referrals, monitor their completion, and maintain proper documentation.
• Maintain accurate patient records in the EHR, upload documentation, and update responsibility notes.
• Monitor task queues and provider inboxes to ensure timely handling of messages, lab results, and follow‑ups.
• Track lab orders and results, ensuring documentation is complete and providers are notified.
• Serve as a communication bridge between patients, providers, labs, pharmacies, and insurance carriers.
• Relay care updates, instructions, and administrative information clearly and accurately.
• Follow up with patients post‑visit to support care‑plan completion and answer administrative questions.

Required profile

• Understanding of insurance verification and prior‑authorization workflows.
• Comfortable working within EHR systems and managing administrative task queues.
• Ability to coordinate referrals, lab work, and multi‑step administrative processes.
• Strong coordination skills across multiple stakeholders (patients, providers, vendors).
• Awareness of HIPAA and data‑confidentiality practices.
• Clear, professional communication with a patient‑first approach.
• Organized, reliable, and proactive in identifying gaps in care processes.
