Virtual Functional Medicine Nurse Practitioner (Full-Time)

About the role

Aligned Modern Health is expanding its telehealth functional medicine practice and seeks licensed Nurse Practitioners to provide virtual, evidence‑based care. The role involves comprehensive patient assessments, personalized treatment planning, and collaboration with a multidisciplinary team to address root causes of chronic conditions.

Key responsibilities

• Conduct thorough patient assessments, including history, physical exams, and functional diagnostic tests.
• Develop and implement individualized treatment plans, prescribing nutraceuticals and medications as needed.
• Monitor patient progress and adjust therapies to ensure effectiveness.
• Collaborate with physicians, dietitians, and other clinicians to coordinate holistic care.
• Maintain accurate electronic health records and ensure compliance with HIPAA regulations.

Required profile

• Minimum two years of experience practicing functional medicine and performing functional diagnostic testing.
• Active Illinois APRN license (or ability to obtain) required; candidates in FL, MI, TN, TX preferred.
• Strong oral and written communication skills and a patient‑first attitude.
• Ability to work independently in a remote telehealth environment.
