Social Worker (MSW) – Full‑time, weekends

About the role

Ochsner Health is seeking a Social Worker to provide bio‑psychosocial assessments, patient education, and crisis interventions for patients and families. The role involves coordinating complex discharge plans and collaborating with a multidisciplinary team to address emotional, social, cultural, and spiritual needs. This position works primarily on weekends and requires flexibility to travel between facilities.

Key responsibilities

• Conduct patient assessment interviews, provide referrals, and coordinate discharge plans.
• Identify and connect patients and families with community resources.
• Maintain accurate patient records and communicate critical information to the medical team.
• Stay current with professional knowledge and required certifications through continuing education.
• Utilize management resources and participate in on‑call rotation.
• Adapt communication style and procedures to respect patient privacy and cultural preferences.

Required profile

• Master’s degree in Social Work (MSW) from a CSWE‑accredited program.
• Certified Social Worker (CSW) or Registered Social Worker (RSW); LMSW preferred or to be obtained within 6 months.
• Computer proficiency for data entry and use of Windows‑style applications.
• Effective verbal and written communication skills.
• Working knowledge of common disease entities, medical terminology, and health‑care systems (Medicare, Medicaid, etc.).
