Senior Integrated Clinic Pharmacist

About the role

We are looking for a Senior Integrated Clinic Pharmacist to join our primary‑care organization and work remotely as part of a multidisciplinary care team. The pharmacist will support a full physician panel, improve medication outcomes for high‑risk patients, and collaborate closely with physicians, nurses and other clinicians.

Key responsibilities

• Apply evidence‑based medicine and clinical judgment to evaluate pharmacotherapy for patients with diabetes, hypertension, heart failure, COPD and other chronic conditions.
• Execute approved clinical protocols and optimize drug coverage, including formulary selection and cost‑effective alternatives.
• Participate in high‑risk rounds, provide patient counseling, and conduct disease‑state education.
• Perform post‑discharge medication reconciliation and document all activities in the clinic EMR and pharmacy management systems.
• Support CMS Stars and HEDIS quality measures, lead quality‑improvement initiatives, and provide in‑service education for staff.
• Stay current with pharmacotherapy guidelines and consult providers on complex medication issues.

Required profile

• Active, unrestricted pharmacist license in Florida (willingness to obtain additional state licenses).
• Pharm.D. or equivalent advanced pharmacy degree.
• Minimum five years of practice experience as a pharmacist.
• Proven ability to work in a team environment and support shared goals.

Required skills

• Proficiency with electronic medical record (EMR) documentation.
• Experience using pharmacy or clinical management systems.
