RN - Weekend Transition Team

About the role

Join BAYADA's Transition Team to provide complex care for infants, children, and adults at home in Philadelphia. You will support the hand‑off from hospital or HICU® services to ongoing home care, partnering with CHOP and training the continued care team.

Key responsibilities

• Deliver high‑quality, patient‑centered care during weekend overnight and day shifts.
• Provide extensive training and education to the ongoing BAYADA care team.

Required profile

• Current Registered Nurse (RN) license.
• NICU, PICU, ICU, or home‑care trach/vent experience.
• Current CPR certification and ability to pass high‑tech adult and pediatric simulations.
• Commitment to a consistent schedule and ability to travel throughout Philadelphia County.

Required skills

• Critical care experience (NICU, PICU, ICU).
• Home‑care tracheostomy and ventilator management.
• CPR certification.
