Remote Post-Acute Nurse Practitioner (Telephonic)

About the role

TeamHealth is looking for a quality‑driven Nurse Practitioner to provide telephonic clinical coverage for skilled nursing facilities. This remote, full‑time or part‑time position offers weekend call coverage, comprehensive onboarding, and the autonomy to manage diverse post‑acute patient needs.

Key responsibilities

• Triaging, implementing, and adjusting individualized care plans based on patient needs.
• Coordinating care to optimize patient outcomes.
• Collaborating with nursing staff, physicians, and other healthcare professionals in a multidisciplinary team.
• Maintaining accurate, timely documentation in compliance with regulations.

Required profile

• Active Nurse Practitioner licenses in multiple states; New York and Maryland licenses strongly preferred.
• Ability to obtain and maintain additional state licenses as needed.
• Experience in post‑acute care and telephone triage.
• Strong clinical foundation and knowledge of healthcare regulatory standards.
• Excellent communication and interpersonal skills.
• Weekend availability.
