Remote Hospital Pharmacist

About the role

Join PharMerica’s dedicated pharmacy team as a Remote Hospital Pharmacist. You will work in a non‑retail environment supporting long‑term care and senior living facilities, providing clinical pharmacy services and ensuring safe medication management.

Key responsibilities

• Evaluate physician medication orders for appropriateness, dosage, interactions, and route.
• Provide clinical recommendations and therapy optimizations to physicians and healthcare staff.
• Maintain up‑to‑date resident medication profiles, including prescription, OTC, and home remedies.
• Order, review, and communicate required laboratory tests for high‑risk medications.
• Collaborate with physicians, nurses, and other professionals to support clinical decisions.
• Document all interventions, communications, and education in the patient’s medical record.
• Implement and enforce policies for safe handling, dispensing, and oversight of medications.
• Ensure compliance with state and federal regulations (CMS, DEA, FDA) and report medication‑related errors.
• Educate staff on safe medication practices and perform additional pharmacy tasks as assigned.

Required profile

• Bachelor’s degree in Pharmacy (Pharm.D. preferred).
• Current, valid pharmacist license in the state of practice.
• At least one year of experience in long‑term care or similar setting.
• Knowledge of drug therapy guidelines, medication safety, and regulatory standards.

Required skills

• Proficiency with Electronic Medical Records (EMR) systems.
• Experience using pharmacy information systems.
