Rehabilitation Manager – PT/OT Home Health

About the role

Join Jefferson at Home by BAYADA as a full‑time Rehabilitation Manager, overseeing a multidisciplinary team of Physical and Occupational Therapists delivering home‑based care in Philadelphia.

Key responsibilities

• Lead and coordinate a multidisciplinary team to ensure high‑quality client care and service.
• Provide clinical guidance, supervision, and ongoing development for field staff.
• Review clinical documentation, including OASIS assessments and care‑plan coding for accuracy and compliance.
• Monitor clinical quality, utilization, and adherence to Medicare Conditions of Participation and agency policies.
• Coordinate case‑management activities such as authorizations, interdisciplinary communication, and client advocacy.
• Participate in recruiting, onboarding, and evaluating field staff.

Required profile

• Current, unrestricted PT or OT license in good standing.
• Minimum two years of community health experience in a Medicare‑certified home health setting.
• Prior clinical and management experience, including supervisory responsibilities.
• Strong knowledge of OASIS, Medicare Conditions of Participation, and home‑health documentation.
• Excellent assessment, case‑management, organizational, and communication skills.

Required skills

• Proficiency with OASIS assessment tools.
• Familiarity with Medicare Conditions of Participation.
• Experience using electronic medical record (EMR) systems.
• Ability to generate and interpret clinical reporting data.
