Registered Nurse – Nurse Navigator

About the role

The Hospital is seeking a Registered Nurse to serve as a Nurse Navigator for patients in Lawrenceville, GA. This role supports patients throughout their care journey, coordinating services and ensuring seamless communication between clinical teams.

Key responsibilities

• Guide patients through the healthcare system, from admission to discharge.
• Collaborate with physicians, case managers, and ancillary staff to develop individualized care plans.
• Provide education on treatment options, follow‑up appointments, and self‑care strategies.
• Monitor patient progress and intervene when barriers to care arise.
• Document all interactions accurately in the electronic health record.

Required profile

• Active, unencumbered RN license in the state of Georgia.
• ADN/ASN degree or higher from an accredited nursing program.
• Experience in roles such as Assistant Nurse Manager, Charge Nurse, Clinical Nurse Coordinator, or similar leadership positions is strongly encouraged.
• Ability to work day, night, or mid shifts on a full‑time, part‑time, or per‑diem basis.

Required skills
