Registered Nurse – Hospital at Home

About the role

We are seeking a compassionate Registered Nurse to join our Hospital at Home program in Park Ridge, IL. You will provide acute‑level care to patients in the comfort of their homes, supporting a range of clinical needs across multiple specialties.

Key responsibilities

• Deliver bedside nursing care to home‑based patients, including assessment, medication administration, and monitoring.
• Manage specialized therapies such as PCA pumps, tube feeding, and wound care.
• Coordinate with multidisciplinary teams to ensure seamless continuity of care.
• Document patient progress accurately and communicate updates to physicians.
• Provide patient and family education on disease management and post‑discharge plans.

Required profile

• Active, unencumbered RN license in Illinois.
• ADN/ASN degree or higher from an accredited nursing program.
• Experience with cardiac, neuro, oncology, orthopedic, or wound care patients is strongly preferred.
• Flexibility to work day, night, or mid shifts on a full‑time, part‑time, or per‑diem basis.
