Registered Nurse - Hospice Admissions

About the role

AdventHealth is hiring a full-time, day-shift Registered Nurse for its Hospice Care unit in Altamonte Springs, Florida. The nurse travels to patients' homes in Orange, Osceola and Seminole counties to assess their physical and emotional condition for hospice eligibility, coordinate admissions and support the hospice team. The schedule is 11:30 am to 10 pm with every other weekend, on 4-10 hour mid-day shifts.

Key responsibilities

• Assess patients' physical and emotional condition for hospice eligibility and communicate available programs and services
• Partner with account representatives to meet patient and hospital staff needs for new referrals
• Obtain required signatures and complete documentation to comply with reimbursement regulations
• Evaluate patients' medical needs, medications and special equipment requirements
• Notify the hospice team of crisis intervention or special services as needed, obtain orders and initiate crisis care documentation
• Obtain approval from hospice and primary care physicians to admit patients and update referral sources on admission status
• Document patient information in the electronic medical record and notify the patient care team by email
• Arrange patient transportation as needed and travel to admitted patients' hospital rooms
• Communicate with departments and the pharmacist about patient equipment and medications

Required profile

• Associate's degree in Nursing (preferred: Bachelor's degree)
• 3+ years of registered nurse experience in home health and/or acute care, including 1 year in a hospital
• Registered Nurse (RN) license, Basic Life Support (CPR) certification and driver's license
• Certified Hospice and Palliative Care RN license preferred

Required skills

• Effective communication in English, reading, writing and typing legibly
• Knowledge of electronic medical records
• Nursing skills performed in line with legal and ethical standards
• Basic to intermediate Microsoft Office (Word, Excel, PowerPoint, Outlook) and internet software
• Reading physician orders, plans of care, policies and procedures
• Following infection control precautions
