Program Aide – Root Cause Analysis Quality Reviewer

About the role

The Program Aide will support Texas A&M University Health Science Center’s Patient Safety Organization by performing comprehensive quality reviews of Root Cause Analysis (RCA) documentation submitted by healthcare facilities. This part‑time, fixed‑term position focuses on identifying gaps in RCA processes and providing consultative feedback to improve patient safety outcomes.

Key responsibilities

• Conduct RCA quality reviews using the RCA Quality Analysis Tool (QAT) and established criteria.
• Evaluate multidisciplinary team composition and subject‑matter expertise.
• Assess flow diagrams, narratives, cause‑and‑effect diagrams, and action plans for completeness and compliance with the Five Rules of Causation.
• Verify root‑cause statements avoid blame‑focused language and adhere to methodological standards.
• Prepare written consultative reports documenting findings and recommendations.

• Familiarity with healthcare quality improvement and patient safety principles.
• Ability to work independently and communicate feedback clearly.
