About the role

Colorado Physician Partners, PLLC is looking for a Primary Care Physician to join its clinic in Aurora, Colorado, one of 12 clinics in an independent private practice in the Aurora area. The practice is part of the independent practice association (IPA) PHPprime, which offers wrap-around services such as care coordination, practice transformation coaching, risk adjustment coding education, population health management, data and analytics, specialist network management and large group contracting.

The clinics are independent, outpatient private practices within the Denver area, with a team of providers and support staff, a social worker and an RN/patient educator who act as care coordinators. Most practices also have mental health support, and physicians hold clinical faculty appointments with the University of Colorado, where medical students and Regis APP students keep their teaching skills sharp. The practice is known for caring for complex patients and has earned maximum scores and bonuses with CPC+.

Required profile

• Board eligible or board certified in Internal Medicine or Family Medicine
• Supervisory skills and interest in working with advanced practice providers
• Geriatric fellowship training or medicine subspecialty interests are welcome
