Post-Acute Physician - Fort Collins, Greeley, Windsor

About the role

Rocky Mountain Senior Care, Colorado's largest post-acute care practice, is hiring a physician for its Northern Colorado team in Greeley, Windsor and Fort Collins. The practice has 100+ physicians, nurse practitioners and physician assistants serving patients in more than 700 facilities, from Colorado Springs to Fort Collins. Positions are available full time or part time.

Key responsibilities

• Care for patients across assisted living, independent living, skilled nursing, memory care and group homes, with a wide range of pathology.
• Work alongside advanced practice providers and lead a team using supervisory skills.
• Take call every other month, which can be traded, picked up extra or taken by others; weekday pay is $200, weekend $275 and holidays $300.

Required profile

• Board eligible or board certified in Internal Medicine or Family Medicine.
• Supervisory skills and interest in working with advanced practice providers.
• Geriatric fellowship training or medicine subspecialty interests are welcome.
