Physician – Post‑Acute/Value‑Based Care (Skilled Nursing)

About the role

The organization partners with physicians to deliver high‑quality, cost‑effective care to medically complex, high‑needs patients in skilled nursing and long‑term care settings. This full‑time physician position provides clinical oversight and direct patient care across multiple facilities, with flexible scheduling and strong support from nurse practitioners and care coordinators.

Key responsibilities

• Provide physician coverage for 8–20 skilled nursing and long‑term care facilities.
• Design and manage a personal rounding schedule for skilled, custodial, and ACO REACH patients.
• Complete required visits per regulatory guidelines (minimum 2 weekly for skilled patients, monthly for custodial patients).
• Participate in daily rounds for ACO REACH patients alongside the medical director.
• Coordinate follow‑up care with NPs, care teams, and the call center.
• Document visits, coding, and discharge summaries in a timely manner.
• Engage in quality improvement, peer review, and utilization management activities.
• Optional paid weekend/overnight call coverage.

Required profile

• MD or DO degree (Internal Medicine, Geriatrics, or Palliative Care preferred).
• Active California medical license, DEA and BLS certifications.
• Strong clinical judgment, documentation, and communication abilities.
• Ability to work independently across multiple facilities.
• Managed‑care experience preferred.
