Physician – Post‑Acute & Senior Living Care (TX)

About the role

Meridian Health Partners is seeking board‑certified physicians to deliver medical care in skilled nursing facilities and senior‑living communities across the Dallas‑Fort Worth area. The position offers both part‑time and full‑time schedules, allowing physicians to work in Glen Rose, Granbury or surrounding locations while providing high‑quality post‑acute and memory‑care services.

Key responsibilities

• Conduct comprehensive patient assessments and develop individualized care plans.
• Provide on‑site medical management for residents in assisted‑living, rehab and memory‑care settings.
• Coordinate with interdisciplinary teams to ensure continuity of care and optimal outcomes.
• Document all clinical activities in the facility’s electronic medical record system.
• Participate in quality‑improvement initiatives and compliance reporting.

Required profile

• Active Texas medical license and DEA certification.
• Board certification in a relevant specialty (e.g., Internal Medicine, Family Medicine).
• Experience in inpatient, hospitalist or post‑acute care settings preferred.
• Commitment to delivering compassionate care to senior populations.
