About the role

Trumen Physicians + Associates is seeking a board‑certified or board‑eligible physician to provide post‑acute care on a supplemental, one‑day‑per‑week basis in skilled nursing and rehabilitation facilities in Tyler, TX. This independent‑contractor role offers autonomy, a flexible schedule, and the opportunity to impact hospital readmission rates and patient outcomes.

Key responsibilities

• Deliver in‑person medical care to patients in designated post‑acute facilities.
• Perform assessments, develop treatment plans, and document care in the EMR.
• Collaborate with facility staff to ensure smooth transitions of care and quality outcomes.
• Participate in Trumen’s value‑based and transitional care programs as needed.
• Maintain an on‑site presence consistent with the agreed schedule.

Required profile

• MD or DO with a current, unrestricted Texas medical license.
• Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatric Medicine.
• 1+ year of experience in post‑acute, geriatric, or SNF care preferred.
• Comfortable practicing independently in skilled nursing facilities.
• Reliable transportation to assigned facilities.
