Physiatrist (PA/NP) for Skilled Nursing Facilities

About the role

Iconic Care is seeking a motivated PA or NP to provide physiatry services in skilled nursing and rehabilitation facilities. You will work independently on‑site, helping patients overcome pain and functional limitations so they can fully engage in therapy.

Key responsibilities

• Round at 1–3 skilled nursing or rehabilitation centers, 1–3 days per week per location.
• Assess up to 25+ patients daily and develop individualized treatment plans.
• Perform targeted interventions such as joint injections, Botox treatments, dry needling, and medication management.
• Collaborate with facility primary‑care teams and refer to other specialties when needed.
• Maintain accurate documentation in the facility’s EHR system.

Required profile

• Current, unrestricted PA or NP license.
• DEA licensure or willingness to obtain it.
• Comfort with performing peripheral joint injections (knees, shoulders, hips).
• Strong communication skills and ability to work autonomously.
