About the role
The Pharmacy Quality Specialist will work within a collaborative healthcare environment to improve patient outcomes through medication safety and quality improvement initiatives. The role combines clinical pharmacy expertise with data‑driven quality programs, supporting evidence‑based practice across the health system.
Key responsibilities
- Partner with healthcare teams to deliver pharmacist‑focused care that enhances safety and patient outcomes.
- Support implementation of pharmacy guidelines, policies, and quality‑improvement projects.
- Collaborate with third‑party payers and stakeholders to ensure adherence to quality measures.
- Promote medication adherence and mitigate risks of high‑risk drugs through patient outreach and education.
- Provide medication‑related data and insights to aid clinical decision‑making.
- Conduct medication reconciliation for patients transitioning to post‑acute care.
- Serve as a pharmacy resource on clinical guidelines and best practices.
- Monitor, collect, and report quality data for performance and regulatory programs.
- Prepare and deliver presentations and continuing‑education sessions for healthcare professionals.
- Participate in research projects focused on medication use, quality improvement, and patient safety.
Required profile
- Licensed pharmacist with experience in clinical pharmacy, healthcare quality, or medication management.
- Strong communication abilities and analytical thinking.
- Proven capacity to collaborate effectively with multidisciplinary teams.
