Pharmacy Nurse Care Coordinator

About the role
The Pharmacy Nurse Care Coordinator provides advanced clinical coordination for patients with bleeding disorders, supporting individualized treatment plans and ensuring seamless collaboration between physicians, pharmacists, and multidisciplinary teams.
Key responsibilities
- Instruct patients on factor dosing, product education, and emergency protocols.
- Coordinate complex cases, triage, and communicate nursing assessments to providers.
- Document patient interactions, refill requests, and care‑coordination activities in the EMR.
- Collaborate with pharmacy and billing departments to meet patient needs and minimize financial risk.
- Work with pharmacists on dosing plans, factor‑utilization reviews, and adherence monitoring.
- Identify care trends, recommend improvements, and conduct home visits and assessments.
- Facilitate factor order completion, non‑factor prescriptions, and home‑infusion training.
- Schedule clinic, follow‑up, and telehealth appointments and support patients in understanding treatment plans.
Required profile
- Valid RN or LPN license in the state of Indiana.
- ASN or BSN degree.
- Residency in Indiana with routine on‑site presence at the Indianapolis center.
- Ability to enable multi‑factor authentication on personal smart devices for system access.
