Pharmacist – Outpatient Pharmacy Services

About the role

The Pharmacist position at Confluence Health provides top‑tier outpatient pharmacy services across multiple sites, including CWH Professional Pharmacy and Tele‑Pharmacy. This pool role supports patient consultations, clinical reviews, drug utilization reviews, and remote verification while ensuring safe, effective medication use.

Key responsibilities

• Deliver patient consultations, clinical medication reviews, and drug utilization reviews for dispensed medications.
• Perform remote verification and supervise pharmacy technicians via Tele‑Pharmacy platforms.
• Provide on‑site backup coverage and therapeutic assessments for individual patients and populations.
• Maintain compliance with hospital policies, formulary management, and therapeutic substitution initiatives.
• Supervise and monitor daily activities of pharmacy technicians to ensure safe and timely pharmaceutical delivery.
• Coordinate discharge medications with hospitalists, discharge nurses, and hospice pharmacists.

Required profile

• Licensed pharmacist with strong knowledge of age‑specific medication issues.
• Ability to apply professional judgment to ensure safe, cost‑effective drug use.
• Excellent critical‑thinking skills for providing drug information to clinicians and patients.
