Pharmacist – Long‑Term Care (Uniondale, NY)

About the role

PharMerica is seeking a full‑time Pharmacist to join its closed‑door pharmacy team in Uniondale, NY. You will work in a non‑retail setting providing medication management for long‑term care and senior living residents.

Key responsibilities

• Evaluate physician medication orders for appropriateness, dosage, interactions and route.
• Provide clinical recommendations to physicians and facility staff.
• Maintain up‑to‑date resident medication profiles, including prescription, OTC and home remedies.
• Order, review and communicate laboratory tests needed for high‑risk medications.
• Collaborate with physicians, nurses and other healthcare professionals.
• Document all communications and interventions in the medical record.
• Enforce policies for safe procurement, storage, administration and disposal of medications.
• Ensure compliance with state and federal regulations (CMS, DEA, FDA).
• Report medication errors and adverse events to the Pharmacy Manager.
• Educate staff on safe medication handling.

Required profile

• Bachelor’s degree in Pharmacy; Pharm.D. preferred.
• Current, valid pharmacist license in the state of practice.
• At least one year of experience in long‑term care (desired).
• Knowledge of drug therapy guidelines, medication safety practices and regulatory standards.

Required skills

• Proficiency with electronic medical records (EMR) systems.
• Experience using pharmacy information systems.
