Pharmacist – Long Term Care (Houston, TX)

About the role
PharMerica is seeking a full‑time Pharmacist to join its closed‑door pharmacy team in Houston, Texas. You will work in a non‑retail setting supporting long‑term care and senior‑living facilities, ensuring safe and effective medication management for residents.
Key responsibilities
- Review physician medication orders for appropriateness, dosage, interactions, and route of administration.
- Provide clinical recommendations and medication therapy optimizations to physicians and healthcare staff.
- Maintain up‑to‑date resident medication profiles, including prescription, OTC, and home remedies.
- Order, review, and communicate laboratory tests required for high‑risk medication monitoring.
- Collaborate with physicians, nurses, and other professionals to deliver clinical support.
- Document all communications, interventions, and education in the medical record.
- Enforce policies for safe handling, dispensing, and oversight of medications.
- Ensure compliance with state and federal regulations (CMS, DEA, FDA) and report medication‑related errors.
- Educate staff on safe medication practices and perform additional pharmacy tasks as assigned.
Required profile
- Bachelor’s degree in Pharmacy; Pharm D preferred.
- Current, valid pharmacist license in the state of practice.
- At least one year of experience in long‑term care pharmacy preferred.
- Strong clinical judgment and effective written and verbal communication.
Required skills
- Knowledge of drug therapy guidelines and medication safety practices.
- Familiarity with regulatory standards (CMS, DEA, FDA).
- Proficiency with electronic medical records and pharmacy information systems.
