Personal Care Pharmacist

About the role
The Personal Care Pharmacist supports the health plan’s Personal Care Pharmacy Team by addressing medication needs of members with multiple chronic conditions. Through telephonic outreach and collaboration with Quality Management, Customer Care, and Care Management, the pharmacist works to close clinical gaps and improve disease‑state management.
Key responsibilities
- Conduct telephonic outreach to members with chronic conditions to identify and close clinical gaps.
- Evaluate medication‑related problems such as adverse events, therapeutic duplication, adherence issues, polypharmacy, and untreated conditions.
- Utilize prescription and medical claims data to develop clinical recommendations that address member‑specific barriers.
- Document, analyze, and report outreach activities, interventions, and outcomes.
- Provide member education, materials, and self‑management resources according to established protocols.
- Resolve benefit issues and refer members to appropriate internal teams.
- Apply formulary and benefit information to guide medication choices and reduce access barriers.
- Collaborate with cross‑functional teams including Quality Management, Provider Relations, Customer Care, and Pharmacy Operations.
- Monitor the drug pipeline to anticipate affordability, access, or utilization barriers.
- Partner with Quality Management and Clinical Strategy to design targeted outreach strategies for high‑impact populations.
- Develop and maintain desk‑level procedures for documentation and outcome measurement.
- Present program outcomes and strategic recommendations to leadership.
- Define program metrics, quantify outcomes, and articulate program value.
- Standardize tools, processes, and best practices to ensure consistency and scalability.
- Mentor and train Personal Care Clinical Pharmacist I and II staff.
- Represent the health plan in regional or national quality‑improvement forums.
