Pediatric Care Manager, Registered Nurse

About the role

AdventHealth South Orlando is hiring a Pediatric Care Manager Registered Nurse for a full-time day shift (Monday to Friday, 8:30 am to 5:00 pm) at its Orlando location. The role focuses on discharge planning, care transitions and advocacy for patients and families.

Key responsibilities

• Take part in multi-disciplinary rounds reviewing patient status, level of care and discharge plans, and escalate care delays to leadership
• Educate patients and families about the emotional, social and financial impact of illness and help them access community resources
• Assess readmitted patients for the reasons behind their readmission
• Organize and run patient and family care conferences with the multidisciplinary team
• Document discharge plans, assessments, barriers to care and patient needs, and coordinate post-acute care authorization with payors
• Review medical records and incorporate clinical, social and financial factors into the care plan
• Develop discharge plans with contingency plans and arrange post-acute and community care
• Use technology and best practices to share patient information with post-acute providers so records are complete and discharge reconciliation is accurate

Required profile

• Registered Nurse (RN)
• Associate degree in nursing (Bachelor's preferred)
• 2+ years of medical or hospital nursing experience
• Prior care management or utilization management experience preferred
• Knowledge of CMS Conditions of Participation for discharge planning

Pay range: $33.08 - $58.04. Applicants must attach their resume to the application; the position is subject to Florida Level 2 background screening.
