Part‑Time Medical Director / Attending Physician (Tyler, TX)

About the role

Theoria Medical is seeking a Medical Director and Attending Physician to provide primary care in skilled nursing facilities (SNFs) in Tyler, TX. The position offers flexible, part‑time scheduling with 1–2 days per week on site, no on‑call or overnight duties, and full autonomy supported by modern technology.

Key responsibilities

• Serve as the attending physician for patients in assigned SNFs.
• Collaborate with nurse practitioners and physician assistants to manage daily patient care and provide clinical supervision.
• Diagnose and treat acute and chronic conditions, delivering patient‑centered primary care.
• Participate in chronic care management, transitional care, remote patient monitoring, and other quality improvement initiatives.

Required profile

• Active U.S. medical license (MD or DO).

Required skills

• Proficiency with Electronic Health Record (EHR) documentation systems.
• Experience using voice dictation and AI‑assisted scribe tools.
• Familiarity with UpToDate clinical guidance platform.
• Ability to engage in remote patient monitoring workflows.
