Part-Time Nurse Practitioner, HouseCalls

About the role

Optum House & Community Care, part of the Optum family, offers a part-time HouseCalls position for Nurse Practitioners or Physician Assistants in Cass, Johnson, Lafayette and Pettis counties, Missouri. The role conducts annual in-home health assessments for Medicare Advantage and other plan members in a non-prescribing, field-based capacity focused on education, gap closure and collaboration with primary care providers. Flexible scheduling is offered between 8am and 8pm over a 7-day week, with no on-call duty, weekends or holidays.

Key responsibilities

• Conduct comprehensive in-home assessments: past medical history review, medication reconciliation, vital signs and physical exam
• Perform evidence-based screenings and point-of-care testing as appropriate
• Identify and document diagnoses for care management and treatment planning
• Communicate findings to members' primary care providers to address gaps in care
• Recognize urgent or emergent situations and intervene appropriately
• Educate members on disease processes, medications and compliance
• Address social determinants of health and provide referrals as needed

Required profile

• NPs: active ANCC or AANP certification in Family, Adult, Geriatric or Adult-Gerontology Primary Care
• PAs: current NCCPA certification and state licensure
• Active, unrestricted state licensure (or ability to obtain it by the start date) without disciplinary actions or practice limitations
• Active, unrestricted driver's license and reliable transportation to complete home visits
• Ability to navigate varied home environments and to sit, stand or kneel as needed
• Preferred: 1+ years of clinical experience (family, geriatric or home health), EMR proficiency, ability to carry equipment up to 30 pounds and climb stairs, proven communication with geriatric or Medicare populations, fluency in another language
