Outpatient Registered Nurse (RN) – Hemodialysis

About the role

The Outpatient Registered Nurse (RN) provides entry‑level, clinically competent care to patients receiving hemodialysis. Working within the End Stage Kidney Disease (ESKD) team, the RN coordinates assessment, planning, intervention, and evaluation for an assigned patient group.

Key responsibilities

• Collect, analyze, and document patient data before, during, and after hemodialysis sessions.
• Adjust treatment plans as needed and notify the Team Leader, Charge Nurse, Supervisor, or Physician.
• Educate patients and families on dialysis, transplant options, and treatment modalities.
• Direct daily safe delivery of hemodialysis therapy, including medication administration.
• Initiate emergency response measures when required.
• Serve as a resource for the care team and assist with staff training and orientation.
• Ensure proper laboratory specimen collection, processing, and shipping.
• Maintain up‑to‑date nursing assessments and care plans in collaboration with the interdisciplinary team.
• Complete Clinical Advancement Program (CAP) requirements for professional growth.

Required profile

• Registered Nurse (RN) license with entry‑level CAP 1 designation.
• Ability to assess, plan, intervene, and evaluate patient care independently and under supervision.
• Strong collaboration and communication skills within a multidisciplinary team.
• Commitment to quality improvement and patient safety.
