Outpatient Physical Therapist – Home Health

About the role

BAYADA Home Health Care is seeking an experienced Physical Therapist to provide outpatient home health visits for senior living communities in the Philadelphia area. The role can be full‑time or part‑time and includes a $5,000 sign‑on bonus.

Key responsibilities

• Make outpatient visits to clients in designated geographic territories.
• Perform diagnostic tests, range‑of‑motion measurements, strength, balance and safety assessments.
• Develop and implement individualized care plans, including therapeutic exercises and gait training.
• Continuously assess and revise care plans and collaborate with nursing staff.
• Educate clients and families on rehabilitative care and independent living.
• Accurately document all observations and interventions using a tablet‑based EMR system.

Required profile

• Current Pennsylvania Physical Therapist license.
• Minimum one year of recent clinical experience as a licensed PT.
• Graduation from an APTA‑approved PT program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational and problem‑solving abilities.

Required skills

• Proficiency with electronic medical record (EMR) systems.
• Solid computer skills for documentation on tablet devices.
