Occupational Therapist - Senior Living Visits

About the role

BAYADA Home Health Care is seeking an experienced Occupational Therapist to provide part‑time OT services in senior living facilities across the Hagerstown, Maryland area. You will join a multidisciplinary team delivering rehabilitative care to adult and geriatric patients living in senior housing.

Key responsibilities

• Make home visits to clients in designated geographic territories.
• Perform comprehensive functional evaluations, including muscle function, endurance, visual coordination, communication skills, and ADL/IADL performance.
• Assess cognitive, psychological and motor aspects, develop individualized intervention plans, and continually revise care plans.
• Evaluate home environments for hazards, identify equipment needs, and recommend adaptations.
• Educate patients, families, and caregivers on rehabilitative activities to promote safety and independence.
• Document observations, interventions, and evaluations using a touch‑pad tablet.

Required profile

• Current Maryland Occupational Therapist license.
• Minimum one year of recent OT experience; prior home‑care experience preferred but not required.
• Graduation from an accredited OT program.
• Ability to work independently, manage time effectively, and demonstrate strong interpersonal and problem‑solving skills.

Required skills

• Proficiency with electronic medical records (EMR) systems.
• Solid computer skills for documentation and tablet use.
