Occupational Therapist – PACE Program

About the role

Habitat Health is expanding its PACE (Program of All‑Inclusive Care for the Elderly) model and seeks an Occupational Therapist to deliver comprehensive OT services to older adults in the Sacramento area. You will work directly with participants in our centers and in their homes, helping them maintain safety and functional independence.

Key responsibilities

• Conduct clinical occupational therapy evaluations and provide treatment across acute, restorative, and maintenance levels.
• Develop individualized treatment plans and document progress in compliance with regulatory standards.
• Perform home visits as needed and record participant and caregiver responses.
• Educate participants, caregivers, and staff on ADL safety, exercise programs, and proper DME use.
• Participate actively in the Interdisciplinary Team, contributing assessments, care‑plan recommendations, and coordination.
• Serve as a subject‑matter expert for OT program standardization, team education, and quality audits.
• Assist with DME inventory management and uphold safety and infection‑control protocols.

Required profile

• Experience providing occupational therapy evaluation and treatment across acute, restorative, and maintenance settings.
• Ability to conduct home visits and document clinical activities accurately.
• Strong collaboration skills to work within an interdisciplinary team.

Required skills

• Knowledge of Activities of Daily Living (ADL) interventions.
• Familiarity with Durable Medical Equipment (DME) usage and safety guidelines.
• Proficiency in clinical documentation and regulatory compliance.
