About the role

Avir Health Group is seeking a Nurse Practitioner to lead clinical care in a Skilled Nursing Facility. You will work closely with the interdisciplinary team to manage both short‑term and long‑term residents, ensuring high‑quality, patient‑centered care.

Key responsibilities

Oversee admissions and discharges of residents.
Conduct monthly stability checks and evaluate changes in condition.
Develop and implement advanced care plans.
Perform comprehensive assessments and prescribe appropriate treatments.

Required profile

Active Texas Advanced Practice Licensure (APN) and DEA license.
Previous nursing or clinical experience in a skilled‑nursing setting.
Strong communication and teamwork abilities.
Master’s degree in Nursing.
