About the role

Our client, a reputable healthcare provider, is seeking a Nurse Practitioner to deliver comprehensive primary care services to homebound patients in Queens and Westchester County. This full‑time position enables you to make a direct impact on patient well‑being by providing care in the comfort of their homes.

Key responsibilities

• Conduct in‑home primary care assessments and treatment for geriatric patients.
• Develop and implement individualized care plans.
• Participate in weekly multidisciplinary team meetings.
• Maintain detailed documentation of patient interactions and treatment plans.
• Manage patient panels to ensure continuity and quality of care.
• Utilize telehealth tools and coordinate with other healthcare providers.
• Travel within designated regions; transportation stipend or Uber reimbursement provided.

Required profile

• Minimum 2 years experience in home care, primary care, or related settings.
• Licensed Nurse Practitioner in New York State.
• Strong clinical assessment and decision‑making abilities.
• Excellent communication and patient engagement skills.
• Reliable transportation; own vehicle preferred (allowance available).
• Ability to work full‑time hours, generally 8 AM to 4 PM.
• Comfort with telehealth technology and electronic health records.
• Bilingual skills are a plus but not required.
