About the role
Join BAYADA's Transition Team as a Licensed Practical Nurse working overnight shifts to support infants, children, and adults transitioning from hospital or HICU® care to home care in Philadelphia.
Key responsibilities
- Deliver high‑quality complex care to patients at home during overnight hours.
- Provide extensive training and education to the ongoing BAYADA care team.
- Coordinate care transitions from hospital or HICU® services to home settings.
Required profile
- Current LPN license.
- Experience in NICU, PICU, ICU, or home‑care pediatric trach/vent settings.
- Current CPR certification and ability to travel throughout Philadelphia County.
Required skills
- Proficiency in high‑tech adult and pediatric simulation assessments.
- Ability to work in a teaching environment and educate care teams.
