Licensed Practical Nurse (LPN)

About the role

Finger Lakes Community Health (FLCH), a Federally Qualified Health Center serving the Finger Lakes Region, is hiring a Licensed Practical Nurse (LPN) in Sodus. The LPN provides the initial screening of patients and assists with screenings, procedures and injections, working with the Registered Nurse to keep the medical clinical area running smoothly and to support patient communication in the electronic health record.

Key responsibilities

• Clinical floor operations: prepare the clinical area for opening and closing, redirect nursing team tasks for efficient patient flow, clean and sterilize instruments and equipment, restock and order medical supplies, and carry out quality control such as monthly control testing and equipment inspections
• Attend the morning huddle with the patient care team, follow up on referrals, and review patient information before visits
• Clinical services: obtain vitals and history, notify the nurse or provider of abnormal findings, assist with minor procedures and exams, perform patient screenings, complete phlebotomy and lab collection, confirm written orders for medications and injections, administer injections and immunizations, and complete medication reconciliation
• Medical record maintenance: scan documents into the patient chart, chart referrals, medications, vaccinations, screenings, lab results and vitals, and enter telephone encounters and referrals in the electronic health record

Required profile

• High school diploma or equivalent
• Current unrestricted NYS LPN license and CPR certification
• Ability to work flexible hours, including some evenings
• Fluent Spanish and English preferred but not required
• Valid New York State driver's license in line with FLCH drivers' policy
• Able to lift up to 40 lbs, sit for long periods, kneel, crawl or bend into awkward spaces, and communicate by phone, web media and in person; OSHA Level 1
