Licensed Medical Social Worker

About the role

Amedisys, part of the Optum family of businesses, provides home health, hospice and community-based services. As a Medical Social Worker you will provide psychosocial support, assessment and care coordination to patients and families receiving services at home, addressing emotional, social, financial and environmental factors that affect health and recovery. You will work closely with the interdisciplinary team to connect patients with community resources, provide counseling and support safe care transitions.

Key responsibilities

• Help patients and families with health-related financial, social and emotional concerns and support effective coping skills
• Provide social work services in line with the plan of care
• Communicate changes in the patient's clinical status to the healthcare team and the care center office in a timely way
• Educate patients, families and team members on social work interventions and the treatment plan
• Perform care planning, transfers and discharge functions
• Identify risks or threats to the patient's health in their environment, act to reduce them and share findings with the physician and the care team
• Other duties as assigned

Required profile

• Masters and/or doctorate degree from a Council on Social Work Education-accredited school of social work
• Active licensure as a Medical Social Worker (LMSW) or Clinical Social Worker (LCSW)
• One year of social work experience in a healthcare setting
• Meets state and federal requirements for the position
• Current CPR certification
• Driver's license and access to reliable transportation
