Intensive In-Home Team Lead

About the role

Join our team to coordinate intake and eligibility processes for individuals using intensive in‑home services. You will lead interdisciplinary teams, develop service plans, and ensure compliance with state regulations.

Key responsibilities

• Conduct intake interviews, complete social histories, and determine eligibility for services.
• Convene admission teams, develop and monitor Individual Service Plans, and track progress toward goals.
• Coordinate funding, identify alternative financial supports, and manage discharge planning.
• Mediate conflicts among providers, families, and individuals, and advocate for client rights.

Required profile

• Master’s degree in Psychology or Social Work and a valid NC license (LCSW, LCSWA, LPC, LCMHC, LCMHCA, LMFT, LMFTA).
• At least one year of documented experience working with the mental health population.
• Knowledge of intensive in‑home policies, clinical diagnoses, and state service definitions.
• Ability to maintain confidentiality, exercise good judgment, and respond effectively in crisis situations.

Required skills

• Understanding of intensive in‑home policies and procedures.
• Familiarity with diagnostic and statistical manuals and human services networks.
• Basic office practices, record‑keeping, and proofreading skills.
