Intensive In-Home Qualified Professional

About the role

The Intensive In-Home Qualified Professional provides family‑focused services directly in the home, supporting consumers and their families with mental health and functional needs. The role involves flexible scheduling to meet the varying demands of each household.

Key responsibilities

• Deliver IIHS services that address the whole family's needs, primarily in the home environment.
• Update individual service plans as consumer needs evolve and coordinate team‑based planning.
• Monitor consumer progress, reassess needs, and evaluate service quality.
• Maintain up‑to‑date crisis plans and respond immediately to crises.
• Document required information promptly and accurately.
• Transition consumers to more appropriate services when necessary.
• Assist families in locating and applying for relevant resources.
• Ensure regular face‑to‑face contact and adjust service intensity based on consumer acuity.
• Report critical incidents and safeguard consumer safety.
• Connect families with community supports and educate them on rights and responsibilities.
• Facilitate monthly team meetings with relevant agencies and community members.
• Provide parenting skills training and support families in managing youth mental health diagnoses.
• Help consumers overcome functional deficits in school, home, and community settings.
• Complete required documentation for Carolina’s Creative Counseling, PA and participate in agency meetings.

Required profile

• Bachelor's degree meeting Qualified Professional Status per NCAC rule 10A NCAC 27G.0104.
• At least one year of documented experience with the target population.
• Successful completion of a nationwide criminal background and driving record check.
• Valid driver’s license, reliable transportation, automobile insurance, and current training.
