Intake Care Coordinator – Substance Use Disorder Treatment

About the role

Indiana Treatment Centers – Columbus is looking for a compassionate and organized Intake Care Coordinator to guide patients through the first hours of treatment. You will create a welcoming, trauma‑informed admission experience while ensuring all clinical, medical, and administrative steps are completed accurately.

Key responsibilities

• Welcome new patients and provide a supportive, trauma‑informed admission experience.
• Conduct initial screenings and biopsychosocial assessments within the scope of your education and licensure.
• Gather detailed information on substance‑use history, behavioral health needs, psychosocial factors, and readiness for change.
• Deliver new‑patient orientation covering program expectations, patient rights, facility guidelines, and treatment expectations.
• Ensure all intake documents, consents, assessments, and electronic health‑record entries are completed accurately and on time.
• Collaborate with nursing, medical providers, therapists, recovery‑support staff, and admissions teams throughout the admission process.
• Coordinate initial physician orders, laboratory work, screenings, and evaluations with the medical team.
• Identify immediate patient needs or barriers and arrange appropriate clinical or operational follow‑up.
• Support the development of preliminary treatment goals and facilitate a smooth handoff to the ongoing treatment team.

Required profile

• Compassionate, highly dependable, and detail‑oriented professional.
• Ability to work collaboratively with multidisciplinary clinical, medical, nursing, and support teams.
• Relevant education, credentialing, and licensure to conduct screenings and assessments.
• Experience in substance‑use disorder or behavioral‑health intake preferred.

• Electronic Health Record (EHR) documentation.
