Hormone Health Nurse Practitioner – Full-Time

About the role

Aligned Modern Health is expanding its Hormone Health practice and seeks licensed Nurse Practitioners to provide Bioidentical Hormone Replacement Therapy (BHRT), thyroid care, and medication management. The position can be remote or based in the Chicago area, focusing on a patient‑first approach and evidence‑based functional medicine.

Key responsibilities

• Conduct comprehensive patient assessments, including history, physical exams, and diagnostic testing.
• Develop and implement personalized treatment plans for hormone health, thyroid disorders, and related conditions.
• Prescribe nutraceuticals and medications, monitor outcomes, and adjust therapy as needed.
• Collaborate with multidisciplinary providers to coordinate care and ensure optimal patient outcomes.
• Maintain accurate electronic health records and stay current with advances in functional, integrative, and lifestyle medicine.

Required profile

• Current APRN license with a concentration in Functional Medicine, Hormone Health, Anti‑Aging, or Lifestyle Medicine.
• Formal training and experience in Bioidentical Hormone Replacement Therapy (BHRT) preferred.
• Active Illinois APRN and DEA licenses (or ability to obtain) for Illinois‑based providers; remote providers must meet state licensing requirements.
• Availability to work at least 16–20 hours per week, including occasional evening shifts.
• Strong communication skills, patient‑first attitude, and adherence to HIPAA confidentiality standards.

Required skills

• Proficiency with electronic health record (EHR) systems.
• Knowledge of HIPAA compliance and patient privacy regulations.
