Home Health Nurse Practitioner – Part‑time

About the role

Our client, a leading home‑health organization, is seeking experienced Nurse Practitioners to conduct Health Risk Assessments for patients in Mobile, AL and surrounding areas. This part‑time, contract position offers flexible scheduling based on patient needs and practitioner availability.

Key responsibilities

• Travel to patients' homes, nursing homes, or other facilities to perform comprehensive history and physical assessments.
• Document acute, chronic, and long‑term health issues accurately using the organization’s EMR system.
• Obtain and file all required consent forms.
• Educate patients and families on chronic conditions, preventive care, and medication adherence.
• Complete referrals to case management, nursing, pharmacy, and other services as needed.
• Participate in clinical meetings and compliance trainings, primarily web‑based.
• Review patient records, interpret laboratory data, and assist in diagnosing medical conditions.
• Perform ADL, fall‑risk, depression, nutritional health screenings, BMI evaluations, and medication reconciliations.

Required profile

• Family Nurse Practitioner certification (Adult NP may be considered if no pediatric patients are seen).
• Board certification by ANCC or AANP.
• Active, unrestricted Maryland NP license (new graduates considered).
• Ability to be credentialed by Medicare, Medicaid, and private insurers.
• Proficiency with computer‑based applications, including MS Office and EMR navigation.
• Access to a personal vehicle with adequate insurance and a reliable home internet connection.
• Strong clinical assessment and analytical skills, capable of working independently.
