Home Health Aide – Personal Care & Support

About the role

Central Star Home Health is seeking compassionate Home Health Aides to provide personalized in‑home care for patients across New York, Ohio, Pennsylvania, and South Florida. You will support patients with daily living activities and ensure a safe, comfortable environment in accordance with individualized care plans.

Key responsibilities

• Assist patients with bathing, toileting, transferring, dressing/undressing, and ambulation.
• Perform household tasks such as making beds, changing linens, laundry, and light housekeeping.
• Plan, prepare, and serve nutritious meals; assist with feeding as directed.
• Document services accurately and promptly, maintaining patient confidentiality.
• Measure and record vital signs (temperature, pulse, respiration, blood pressure) when ordered.
• Report any changes in patient condition or incidents to the RN Supervisor or therapist.
• Carry out additional duties assigned by supervisory staff.

Required profile

• Current certification/registration required by state and/or federal regulations for home health services.
• Ability to follow individualized care plans and maintain strict confidentiality.
• Strong observational skills to notice and report changes in patient status.
• Willingness to work in the specified states and travel to patient homes.

Required skills

• Vital signs measurement (temperature, pulse, respiration, blood pressure).
