Home Health Aide

About the role

We are seeking compassionate Home Health Aides to provide personalized in‑home care for patients across New York, Ohio, Pennsylvania and South Florida. You will support patients with daily living activities and ensure their comfort and safety in accordance with individualized care plans.

Key responsibilities

• Assist patients with bathing, dressing, transferring, ambulation and toileting.
• Perform light housekeeping tasks such as making beds, changing linens, laundry and general cleaning.
• Plan, prepare and serve nutritious meals and assist with feeding as directed.
• Document services accurately and promptly, maintaining patient confidentiality.
• Measure and record vital signs (temperature, pulse, respiration, blood pressure) when ordered.
• Report any changes in condition or incidents to the RN supervisor or therapist.
• Carry out additional duties as assigned and comply with state/federal certification requirements.

Required profile

• Current state‑required certification or registration for Home Health Aide services.
• Ability to follow individualized care plans and maintain patient confidentiality.
• Willingness to work in the listed states and travel to patient homes.
• Strong attention to detail and timely reporting of observations.
