Geriatric Advanced Practice Provider – Full‑time

About the role

The Dorothy Adler Geriatric Assessment Center is seeking a full‑time Advanced Practice Provider (NP or PA) to join its high‑volume outpatient geriatrics team. You will serve as a core clinical provider, delivering comprehensive assessments and ongoing care to a growing population of older adults.

Key responsibilities

• Conduct comprehensive geriatric assessments for new and follow‑up patients.
• Manage complex medical, cognitive, and functional needs of older adults.
• Maintain a panel of patients across approximately seven clinic sessions per week.
• Collaborate daily with a multidisciplinary team to coordinate care for patients and families.

Required profile

• Licensed Nurse Practitioner or Physician Assistant with interest or experience in geriatrics.
• Strong clinical judgment and ability to manage complex patients independently.
• Team‑oriented mindset with a commitment to patient‑centered care.

Required skills

• Proficiency with EPIC EMR system.
