Dialysis Social Worker (MSW) – Full‑time

About the role

Dialysis Clinic, Inc. is seeking a compassionate MSW‑qualified Social Worker to join our nonprofit dialysis team. You will provide psychosocial support to patients with chronic kidney disease, helping them navigate emotional, financial, and lifestyle challenges while promoting optimal health outcomes.

Key responsibilities

• Assess and address the emotional, financial, and social needs of dialysis patients.
• Connect patients with community resources for nutrition, transportation, housing, and counseling.
• Advise patients and families on treatment options, including dialysis, transplant, and medical management.
• Coordinate translation services and advocate for patient access to healthcare benefits.
• Develop individualized care plans with the interdisciplinary team and adjust them as needs evolve.
• Provide counseling to support coping, treatment adherence, and rehabilitation goals.

Required profile

• Active, unrestricted U.S. Virgin Islands social work license.
• MSW degree and experience in renal or chronic illness social work preferred.
• Full‑time availability Monday‑Friday, 8 am start, weekends off.
• Strong interpersonal skills and a commitment to patient‑centered care.
